B细胞激活因子(B-cell activation factor,BAFF)又称BLyS、zTNF4、TNFSF13B、THANK和TALL-1,是TNF样细胞因子家族中的一员。BAFF与三种受体结合:b细胞激活因子受体(BAFF-r)、跨膜激活剂-钙调剂-亲环配体相互作用剂(TACI)、b细胞成熟抗原(BCMA)。

BAFF与它的受体之间的相互作用触发对于B细胞的形成和维持来说是必要的信号,进而响应外来物质的侵袭合成免疫球蛋白。在患者中适当的BAFF有助于维持正常的免疫力水平,而不足的水平可能会引起免疫缺陷,但过高的水平可能会引起异常高的抗体产生。当患者表现出自身免疫时,它产生针对自身组织的抗体。系统性红斑狼疮和类风湿关节炎都是由于体内较高水平的BAFF引起的。因此,调节BAFF的水平对于自身免疫疾病的治疗相当重要。

Belimumab是一种靶向可溶性BAFF的人类抗体,它的成功开发标志着系统性红斑狼疮(SLE)生物治疗发展的一个重要里程碑。Lanalumab是一种新的BAFF靶向抗体,通过阻断B细胞上的BAFF受体来阻止BAFF信号,该药物在干燥综合征临床试验中显示出很好的疗效,目前正在开展治疗SLE的临床Ⅱ期试验。
